Complete Physicals for Every Need

School Physicals – Students in grade K-12 who are starting school in Florida for the first time must present a record of a physical exam completed within the past 12 months. A specific form is not required, but it is recommended that you use Florida’s standardize School Entry Exam From. This form must be completed by a healthcare provider who is licensed in the US to perform physical exams.

 

Sports Physicals – Students who wish to participate in interscholastic athletics mush have a pre-participation physical exam (PPE) before they can play. The purpose of PPE is to helps determine whether or not a child (or adult) is physically able to participate in a sport safely.  Most states require that kids and teens have a sport physical before they can start a new sport or begin a new competitive season. The two main parts to a sports physical are the medical history and the physical exam.

DOT Physicals  – The DOT physical exam is a necessary test for any individual hoping to get behind the wheel of a commercial motor vehicle (CMV). This examination makes sure that all commercial drivers are up to par in their daily health and able to handle the long hours and stress of operating a commercial vehicle. It takes special certification for a provider to administer DOT physical exams and it is a very thorough exam to ensure the safety of the American people.

When attending your DOT physical exam make sure you have a completed medical examination report. You will also need to bring any documents listing medications which has been administered by a doctor in the past month.
